Reconstructive Surgery is performed to correct abnormal structures of the body which are caused by congenital birth defects, trauma, developmental abnormalities, infection, tumors or disease.<br />
Rhinoplasty Plastic Surgery A Brief History<br />
Plastic surgery is a surgical specialty dedicated to reconstruction of facial and body defects due to birth disorders, trauma, burns and disease. The word &#8220;plastic&#8221; comes from the Greek word plastikos, which means &#8220;to mold or shape.&#8221;<br />
This dates back to ancient Egypt around 3,000 B.C.  and India since 800 B.C. Soldiers wounded in battle incurred many gaping wounds including facial wounds &#8211; that surgeons were forced to deal with. During the Renaissance, Rhinoplasty was studied for aesthetic purposes and by the middle 1800s doctors in Europe were using different surgical procedures to enhance the shape, appearance and functionality of the nose.<br />
Two of the most noted pioneers were Jacques Joseph and John Orland who began to explore the benefits of &#8220;reconstructive&#8221; and &#8220;functional&#8221; repairs and developed successful surgical techniques to perpetuate this area of modern surgery. During World War I, surgeons were forced to create major advances, quickly, in technologies, reconstructive procedures, anesthesia and techniques. Rhinoplasty was one of the most common surgeries needed and wonderful advancements were made in technique and reduced risk.<br />
So the &#8220;art&#8221; and &#8220;science&#8221; of plastic surgery was born out of necessity and has evolved into cosmetic and reconstructive plastic surgery as we know it today.<br />
Common problems of the Nose<br />
Rhinoplasty surgery is one of the most common cosmetic surgeries performed today. People choose to have their nose reshaped for many reasons. If you can identify with any of the following then lets talk about your needs:<br />

			<content:encoded><![CDATA[<p>Rhinoplasty, also called &#8220;nose refinement surgery,&#8221; is the most complicated cosmetic surgery procedure performed today. The procedure is able to reduce or increase the size of your nose, adjust the shape of the nose&#8217;s tip or bridge, make the nostrils smaller and closer together, or adjust the angle between your nose and upper lip. Rhinoplasty may also be able to correct birth defects or injuries, potentially relieving some breathing problems.<br />
The best candidates for Rhinoplasty are people who are looking for improvement in the way they look, are physically healthy, psychologically stable, and have realistic expectations. During the initial consultation, the surgeon will evaluate the structure of your nose and face and discuss the possibilities for enhancement with you. The doctor will also explain some factors that can influence the procedure and results. Some of these factors include your cartilage and nasal bones&#8217; structure, the shape of your face, thickness of your skin, your age, and your expectations regarding the procedure.<br />
Be sure to tell the surgeon if you have had any previous nose surgery or an injury to your nose. You should also inform your surgeon if you have any allergies or breathing difficulties, if you are taking any medications, vitamins, or recreational drugs and if you smoke.<br />
You will be provided specific instructions regarding how to prepare for the surgery, guidelines on eating and drinking, smoking, taking or avoiding certain vitamins/medications, and washing your face. Carefully following these instructions will help your surgery go more smoothly.<br />
Rhinoplasty usually takes an hour or two, though complicated procedures may take longer. During surgery, the skin of the nose is separated from its supporting framework of bone and cartilage, which is then sculpted to the desired shape. The nature of the sculpting will depend on your problem and the Surgeon&#8217;s preferred technique. Finally, the skin is redraped over the new framework.<br />
Most physicians place nasal packs or soft plastic splints in your nostrils, a procedure perceived by many patients as unpleasant and uncomfortable. Surgeons use the most advanced surgical techniques, allowing them to avoid packing the nose with gauze in most of the cases. When the surgery is complete, a splint will be applied to help your nose maintain its new shape.<br />
After surgery, especially during the first twenty-four hours, your face will feel puffy, your nose may ache, and you may have a dull headache. You can control any discomfort with the pain medication prescribed by the surgeon. On the first day, you&#8217;ll need to stay in bed and keep your head elevated.<br />
You will notice that the swelling and bruising around your eyes will increase at first, reaching a peak after two or three days. The application of cold compresses can reduce the swelling, as well as make you feel a bit better. Most of the swelling and bruising should disappear within two weeks.<br />
Some minor bleeding may occur during the first few days after the surgery. You may continue to feel some stuffiness for several weeks. The surgeon will ask you not to blow your nose for a week or so, while the tissues heal. After 7 to 10 days, all dressings and stitches will be removed.<br />
Most rhinoplasty patients are up and about within two days, and able to return to school or work a week following surgery. The surgeon will give you specific guidelines for gradually resuming your normal activities, but generally you should avoid strenuous activities for two to three weeks. You should also avoid hitting or rubbing your nose, or getting it sunburned. Be gentle when washing your face and hair or using cosmetics.<br />
Day by day, your nose will begin to look better and your spirits will improve. Even though it will take months for the final results to emerge, you will be able to enjoy your new look a week or two after surgery. </p>

			<content:encoded><![CDATA[<p>You may be a candidate for nose reshaping (rhinoplasty) in Los Angeles if you have a nose that is cosmetically unattractive or does not &#8220;fit&#8221; with other facial features. Rhinoplasty may also be indicated for abnormal appearance of the nose as a result of birth defects, injury, or disease. </p>
<p>Nose reshaping is intended to produce a more attractive nasal shape and/or a nose in better proportion to other facial features. Rhinoplasty is done on an outpatient basis under sedation, and local or general anesthesia. Your plastic surgeon will decide which of the following techniques should be used: </p>
<p>STANDARD RHINOPLASTY: Incisions are made inside the nostrils. The nasal bones, cartilage, and soft tissues are reshaped to improve the external contour. </p>
<p>OPEN RHINOPLASTY: Frequently used for difficult or larger noses. Involves one external incision across the columella (strut between the nostrils). Bones, cartilage, and soft tissues are reshaped under direct vision. </p>
<p>TIP-PLASTY: If only the nasal tip is abnormal, it may be corrected with either a standard or external (open) approach. </p>
<p>Large nostrils may be reduced through tiny incisions at the outer base of the nostrils with either type of rhinoplasty.</p>
<p>The patient goes home with an external cast or splint in place. The nose may be packed for 24-48 hours. The cast or splint is usually removed in 7-10 days. Initial discomfort is easily controlled with oral medication. The majority of swelling and bruising subsides progressively over 2-4 weeks. External sutures (if any) are removed in 4-6 days. Internal sutures will dissolve. </p>
<p>The semifinal result of a nose reshaping procedure is evident in 3 months. The final result is evident in 12-14 months. An additional procedure that may enhance the result is a chin enlargement. If you need to improve your breathing, correction of a deviated septum (Septoplasty) may also be appropriate. Rhinoplasty is not covered by insurance if being done for cosmetic reasons. If however, the nasal deformity is due to recent injury, insurance coverage may be available. Pre-authorization information can be obtained from your insurance company. </p>
<p>The specific risks and the suitability of nose reshaping for a given individual can be determined only at the time of consultation. All surgical procedures have some degree of risk. Minor complications that do not affect the outcome occur occasionally. Major complications are unusual. </p>

			<content:encoded><![CDATA[<p>One of the real concerns of many patients who are considering rhinoplasty surgery, I have learned, is that their nose will be packed after surgery.the removal of nasal packing is a very uncomfortable thought for most patients and one that they would like to avoid.</p>
<p>The thought of ansal packing removal is unpleasant at best and downright painful at worst. I have found that many potential rhinoplasty patients often ask&#8230;.are you going to pack my nose? After hearing this concern numeruos times over the years, I have come to appreciate a patient&#8217;s apprehensions over the issue of packing in the nose after rhinoplasty surgery.</p>
<p>The good news is that the need for packing a rhinoplasty surgery patient is very infrequent. In cosmetic rhinoplasty, where only the visible part of the nose is being changed, there is very little reason to ever have to pack the nose after surgery as this causes very little internal bleeding. If your nose surgery is to improve your breathing, known as functional nasal airway surgery, nasal packing may have to be used if there is persistent bleeding still at the end of the operation. My experience, however, is that this is not very frequent.</p>
<p>Nasal packing achieves two purposes; 1) to manage bleeding during and after surgery, and 2) to push the lining back into its proper place against the bone or cartilage walls of the internal nose. When straightening the septum, which is often necessary to improve nasal breathing, the lining of the nose must first be lifted up. The lining of the septum must be put back after it is straightened. Packing of the nose has been traditionally used to push this lining back into its original place. The use of packing has been replaced by many surgeons with sewing it back into place through a sewing technique known as quilting. Much to the happiness of patients and plastic surgeons alike, this internal suturing method has replaced the need for nasal packing in most cases. Only in cases where there is too much bleeding at the end of surgery will packing be used to control it, much like the way it is used to stop difficult nose bleeds.</p>
<p>So to those patients considering most forms of rhinoplasty surgery, this is one issue that you need not overly concern yourself with! Modern rhinoplasty techniques have virtually eliminated the need for this uncomfortable consideration. Not only is the concern about pain associated with packing removal eliminated, but the fear of not being able to breathe through your nose at all ( a claustrophobic sensation) right after surgery is also alleviated. </p>
